Creating and reviewing goals

A goal describes an outcome that matters to the person receiving support. Link tasks and documents to show the work behind it, and review the goal as the real situation changes.

Who this article is for

Care-team members who plan, own or follow shared outcomes

Where to find it

  • Open Goals inside the care-team workspace and choose Create goal.
  • Professionals can also choose Goal from Create on the Tasks page after selecting a client, if their role permits it.

Describe the outcome first

  1. 1

Give the goal a clear title

Describe what you want to achieve, such as “Build confidence joining a weekly community activity”.

  2. 2

    Explain why it matters

Add a short description in the person’s own words where possible. Choose an owner or target date if helpful.

  3. 3

    Save the goal

Check the care team and details. New goals begin at Not started unless you choose another status.

  4. 4

    Add a practical next step

Link a task such as “Ask about two suitable local activities”. A goal can have several tasks, each with its own responsible person.

Simple goal examples

  • Get a new wheelchair and seating supports approved.
  • Build confidence joining a weekly community activity.
  • Make morning routines safer and more independent.

Work through the goal action plan template before entering the agreed work.

Use an honest goal status

  • Not started — meaningful work has not begun.
  • Under way — the care team is actively working toward the outcome.
  • Waiting — progress depends on somebody or something else.
  • At risk — the outcome needs attention because timing, ownership or another issue may prevent it.
  • Achieved — the desired outcome has genuinely been reached.

Goals are not calculated percentages

Kinovo shows exact linked-action counts instead. Completing every linked task does not prove that a real-life outcome has been achieved, so Achieved is a deliberate decision.

Check progress with the person and team

Open the goal to see the owner, target date, current status, latest update and linked tasks or documents.

Use the next action to decide what needs attention, then check whether the goal still reflects what the person wants. Change the plan when their priorities or circumstances change.

Keep the outcome current

  • Update the status when the real situation changes.
  • Change the owner or target date when responsibility or timing changes.
  • Link new tasks instead of placing every operational detail in the description.
  • Mark the goal achieved only when the outcome itself has been reached.
  • Leave goals optional—ordinary tasks, documents and Care Protocols can still be useful without one.

If you cannot create or edit a goal

Check Team → My permissions or ask the Team leader. Kinovo may let you propose a goal for approval when you cannot publish one directly.

You do not have to create a goal before using tasks, documents or Care Protocols. Link them only when the connection is useful.
